Our normal opening hours are 8.30am to 6.00pm, Monday to Friday

Outside of these hours, if you have an urgent medical need which cannot wait until the surgery reopens, please ring NHS 111 where you will be assisted to receive the right care.

In an emergency when someone needs medical help to save their life dial 999.

 

Travel Information

If you require any vaccinations relating to foreign travel you need to download and complete this travel questionnaire which will enable the nurse to prepare for your consultation.

Please take the completed and signed travel questionnaire to the practice reception and they will be happy to make a 20 minute appointment with Julia Gould-Smith, Practice Nurse to discuss your travel arrangements. 

It is important to make this initial appointment as early as possible  - at least 6 weeks before you travel - as a second appointment may be required with the practice nurse.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.
